Incident Learning Review – More Than Meets The Eye

After a workplace incident occurs, it is important to understand why it occurred in order to prevent something similar from happening again. However, the traditional approach—examining the incident in a linear fashion and trying to identify what human error led to the event—does not fix the problem. Instead, we should change our approach to better understand the factors and conditions that led to the incident, so we can create sustainable change in the organization. Learn how to:

• Understand the system in place, to better understand the incident that took place
• Avoid simply blaming the worker
• Identify what went wrong with the system that led to the incident
• Build a better model to create sustainable safety
